Summary
This study is testing a new support program for people who have trouble taking their blood pressure medicine as prescribed. It pairs a pharmacist with a community health worker to help patients overcome personal and practical barriers to taking their medications. The goal is to see if this team approach improves medication use and lowers blood pressure, especially for African-American and Latino patients.
