Summary
This study tests whether having people with advanced chronic kidney disease report their symptoms each month using an electronic questionnaire can improve their quality of life. About 70 adults will either use the symptom tracker or receive standard care. The goal is to see if this approach helps doctors better recognize and treat symptoms.
