Can a simple symptom tracker improve life for kidney patients?

Summary

This study tests whether having people with advanced chronic kidney disease report their symptoms each month using an electronic questionnaire, with results shared with their kidney doctor, can improve quality of life and symptom management. About 70 adults with stage 4 or 5 kidney disease will be randomly assigned to use the symptom tracker or receive usual care for 12 months. The goal is to see if this approach is practical and helpful for both patients and doctors.
