Can a simple care plan improve life after stem cell transplant?

Summary
This study tests whether a survivorship care plan is practical for blood cancer patients who have had a stem cell transplant. About 40 patients and their caregivers will take part, answering questions about quality of life and needs. The goal is to see if the plan works within Italy's health system, not to cure the disease.
