New program aims to boost stroke recovery at home

Summary
This study tests a support program to help stroke survivors better manage their health after leaving the hospital. About 140 adults who have had a stroke will take part. Half will get usual care, and the other half will also attend 5 weekly sessions to learn self-management skills. The goal is to see if the program improves their confidence, independence, and stroke knowledge.
