Summary
This study tested whether a personalized, long-term follow-up program could help stroke survivors maintain their independence and prevent functional decline. 301 participants who had a stroke were randomly assigned to receive either this new program or standard care. The program involved monthly check-ins with a stroke coordinator for 18 months to create and adjust a personal plan targeting physical, cognitive, and lifestyle needs.
