Summary
This study tests whether a remote coaching program can help people recover from an ischemic stroke. For three months, 60 participants will receive phone calls, videos, and support to build confidence in managing their health, improve lifestyle factors like diet and blood pressure, and avoid going back to the hospital. The goal is to see if this convenient, at-home support leads to better long-term health outcomes.
