Summary
This study works with stroke survivors, their carers, and healthcare staff to create a program that helps people stay physically active after finishing rehab. Many stroke survivors struggle to keep exercising on their own, which raises their risk of another stroke. The goal is to design and test a support plan that makes long-term activity easier and more sustainable.
