Summary
This study tests a strength-based exercise program that seniors can do at home after leaving the hospital. The goal is to help them regain physical function and reduce frailty. Researchers will enroll 256 people aged 60 and older who are at risk of losing independence after a hospital stay. Participants will follow a tailored exercise plan, and their progress will be measured through physical tests and surveys.
