New toolkit aims to ease tough hospital transfer choices for nursing home residents

Summary
This study tests whether a toolkit called STEP helps long-term care residents, their families, and nurses make better decisions about whether to go to the hospital during a sudden health problem. About 200 residents and their care partners will use the STEP tool during care planning and emergencies. Researchers will compare surveys before and after using the tool to see if it reduces confusion and improves confidence in decision-making.
