Summary

This study tests a toolkit called STEP that helps long-term care residents, their families, and nurses make better decisions about whether to go to the hospital during a health crisis. About 200 residents and their care partners will use the toolkit and complete surveys to measure how confident they feel about their choices. Researchers will compare results before and after the toolkit is used to see if it reduces confusion and improves decision-making.
