Summary

This study is testing if connecting lonely older adults with diabetes to a social worker can help them feel less isolated and better manage their blood sugar. Researchers will compare 90 seniors who receive social worker support to those who continue with their usual diabetes care. The main goal is to see if this social support leads to lower loneliness scores and improved A1C blood sugar levels after six months.
