Summary
This study tested whether having a community health surveyor reach out to patients makes them more likely to complete a questionnaire about their social needs, like income or housing. About 760 adult patients from a family health team who were considered hard-to-reach took part. The goal was to see if this personal support helps clinics better understand and address health disparities.
