Summary
This study compares getting skilled nursing care at home versus in a traditional facility for people who need rehabilitation after a hospital stay. About 300 adults will be randomly assigned to either recover at home with a technology-supported care team or go to a skilled nursing facility. The main goal is to see if home care leads to similar or better improvements in daily activities.
