Summary
This study aims to make diabetes care safer by helping patients switch from older, riskier pills (sulfonylureas) to newer, safer medications. Researchers will give some patients a simple list of questions to ask their doctor about medication risks and alternatives, while others get general information. The goal is to see if this approach leads to more conversations about medication safety and more people switching to better options.
