Summary

This study tests a rehabilitation program combining daily low-flow dialysis with exercise and nutrition monitoring for kidney patients who recently left the hospital. The goal is to improve muscle strength and physical function, which often decline in people on standard dialysis. About 50 participants will be enrolled to see if this approach is more effective than usual care.
