Heart Patients' symptoms tracked remotely in new comfort care study

Summary
This study is testing whether a remote monitoring system can help identify when heart failure patients might benefit from comfort-focused (palliative) care. About 90 adults with worsening symptoms will answer a digital questionnaire about their needs over 4 weeks. The goal is to see if this remote approach is practical and acceptable for patients and doctors, not to test a new drug or cure.
