Summary

This study is testing whether team-based video visits and home monitoring devices can help people better manage chronic conditions like diabetes and high blood pressure. It will involve 40 adults who are already part of a health program and have these conditions. The main goal is to see if this remote approach improves people's confidence in managing their health and their ability to follow medical advice over 16 weeks.
