Summary
This study tests a program for adults with type 2 diabetes who are in the hospital and have unmet social needs (like food or housing). The program screens for these needs at the bedside and provides a navigator to help after discharge. Researchers will see if this reduces emergency visits and readmissions within 90 days compared to usual care.
