New tool aims to keep people with mental health conditions out of the hospital

Summary
This study is testing a tool called PRPP to see if it helps care teams make better decisions about supporting older adults with long-term psychiatric conditions. The goal is to help these individuals continue living in their own homes safely or find the most suitable living arrangement. Researchers will follow 66 participants to see if using this tool improves daily task performance and reduces hospital stays.
