Can a personal hospital team reduce repeat stays for the sickest patients?

This study looks at whether having a special hospital team that gets to know frequently hospitalized patients can improve their care. About 1200 adults who have been hospitalized two or more times in the past year will take part. Researchers will compare those assigned to the dedicated team with those getting usual care to see if they are more likely to see the same team on future visits.
