Summary
This study looks at whether having a special hospital team that gets to know patients who are frequently hospitalized can improve their care. Researchers will compare patients assigned to this team with those getting usual care. The main goal is to see if these patients are more likely to see the same team when they come back to the hospital. Some patients or their caregivers will also be interviewed to better understand their needs.
