Summary
This is a small pilot study in Toronto to gather information for a future, larger national trial. It aims to see if a trained 'Primary Care Connector' who knows the local health system can help vulnerable adults without a family doctor find one more effectively than just giving them written information. Researchers will compare the two methods by seeing how many people successfully get connected to a primary care provider.
