Summary
This study tested whether adding a clinical pharmacist to the care team could reduce medication-related problems when patients move from a hospital to a skilled nursing facility. Over 1,000 adults participated. The pharmacist reviewed medications and coordinated with staff to catch errors. The goal was to see if this approach leads to fewer drug mistakes, hospital readmissions, and deaths within 30 days.
