Can better doctor training save diabetic kidneys?

Summary
This study aims to see if special training for family doctors can help slow the loss of kidney function in older adults with long-term type 2 diabetes. It will compare 360 patients whose doctors receive this training to a group receiving usual care. The main goal is to measure if the rate of kidney decline improves over two years.
