Summary
This study tested whether a tailored program of exercise, cognitive training, and support could help stroke survivors avoid losing function over time. 301 people who had a stroke took part. The program was designed to address each person's specific challenges, such as fatigue, mood, or mobility issues. The goal was to see if this approach could keep people independent and prevent further decline.
