Summary
This study tested whether a tool inside a patient portal could help people with mild memory problems discuss and document their future healthcare wishes. About 300 older adults with mild cognitive impairment or early dementia took part. The goal was to see if they would use the tool and if it led to more advance care planning with their doctors.
