Summary
This study tested a program called PICTURE-THIS that helps people who survived a stay in the intensive care unit (ICU) and their families transition home. The program includes care coordination and activity-based rehab delivered by a specialist team. The goal was to see if the program is practical and acceptable to patients and families, not to measure health outcomes. 42 participants were enrolled.
