Summary
This study looks at whether an enhanced community health worker program can help 500 Philadelphia residents aged 35-75 with heart disease risk factors better manage their health and address social needs like housing or food. Participants will either join group sessions plus one-on-one talks with a health worker or receive standard services. The goal is to improve self-care skills and reduce health gaps.
