Summary

This study tested if adding a clinical pharmacist to the discharge team could prevent medication problems when patients move from a hospital to a skilled nursing facility. Over 1,000 adult patients were involved. The pharmacist worked to coordinate care between the hospital and nursing home, review and adjust medications, and monitor patients after discharge.
