Summary
This study aimed to understand the challenges Spanish-speaking patients face when leaving the hospital for a nursing home. Researchers interviewed 17 patients and their caregivers to learn about communication barriers and what helps or hurts a smooth transition. The goal was to gather patient stories to improve future hospital discharge processes and make them more equitable for people with limited English.
