Summary

This study looked at whether having a pharmacist talk with patients at home after a hospital stay could reduce flare-ups of chronic obstructive pulmonary disease (COPD). 174 adults with moderate to severe COPD took part. The main goal was to count how many times patients had worsening symptoms like coughing or trouble breathing.
