Summary
This study tested whether having a pharmacist talk to patients about life challenges like housing, food, or transportation after an emergency room visit could help them stay healthier and avoid returning to the hospital. About 150 patients were involved, with some receiving this screening and referrals to community help, while others did not. The main goal was to see if this support could reduce unplanned trips back to the hospital or ER within a month.
