Summary
This study looks at whether adding a clinical pharmacist to community mental health teams (called FACT teams) can help people with severe mental illness and substance use disorders feel better and manage their medications more effectively. About 160 adults will take part, and the pharmacist will review medications, counsel patients, and work with the team to solve drug-related problems. The main goal is to see if this improves patients' quality of life.
