NCT ID NCT02887001

Summary

This study tested whether adding a clinical pharmacist to hospital care teams improves patient safety and reduces costs. It involved 500 patients admitted to infectious disease or internal medicine wards. The goal was to see if the pharmacist could help prevent medication errors during hospital stays and transitions, which can lead to longer hospitalizations and higher expenses.
