Summary

This study tests a team of pharmacists and community health workers (CHWs) to help people with high blood pressure take their medications as prescribed. The goal is to improve medication adherence and lower blood pressure. The study includes 217 adults who identify as African-American or Latino, take at least 5 chronic medications, and have low adherence to blood pressure medicine.
