Summary

This study tests whether having pharmacists help manage medications during and after a hospital stay can prevent patients from needing to return to the hospital or emergency room within 30 days. It focuses on older adults (over 55) who take many medications or certain high-risk drugs. The goal is to see if better medication support at discharge leads to fewer unplanned return visits.
