Summary

This study tests whether personalized support from a sports and health professional after cardiac rehabilitation helps people with heart failure stay physically active for a full year. About 90 adults with stable heart failure will either receive this tailored follow-up or standard care. The goal is to see if the extra support helps maintain the benefits of rehab, like better fitness and quality of life.
