Can a personal coach help heart patients keep their gains?

Summary
This study is testing if personalized follow-up support from a sports and health professional helps people with chronic heart failure maintain the benefits of their cardiac rehabilitation program. It will compare 90 patients who receive this personalized coaching for a year after rehab to a group who receive usual care. The main goal is to see if the coached group stays more physically active and keeps their improved fitness and quality of life longer.
