Summary

This study looks at whether personalized support from a sports and health professional can help people with heart failure stay physically active for a full year after completing a cardiac rehab program. About 90 adults with stable heart failure will either receive this tailored follow-up or standard care. The goal is to see if the extra support helps them maintain a higher daily step count and better quality of life over time.
