Personalized coaching after heart rehab may keep patients moving longer

Summary

This study looks at whether personalized support from a sport and health professional can help people with heart failure stay physically active for a full year after completing a cardiac rehab program. About 90 adults with stable heart failure will either receive this tailored follow-up or standard care. The main goal is to see if those with personalized support take more daily steps after 12 months.
