Summary
This study will test whether a new smartphone app helps people recently released from prison manage their health information. Researchers will enroll 100 adults with at least one chronic condition who have been out of jail for less than a year. The goal is to see if the app is easy to use and whether former inmates actually use it to keep track of their health.
