Summary

This study is testing a program designed to improve end-of-life care for frail elderly residents in long-term care homes. The program trains staff, provides education to families, and aims to help residents stay comfortably in their care home rather than being transferred to hospitals. Researchers are measuring whether the program increases family satisfaction with care and reduces emergency hospital visits during residents' final year of life.
