Summary
This study tested a program for people with heart failure that combines remote monitoring (tracking symptoms and weight at home) with nurse education and a hospital check-up. The goal was to see if this approach could reduce hospital stays and deaths. The study included 504 older adults who had recently been hospitalized for heart failure.
