Summary
This study looks at whether a pharmacist-led program can help people with heart failure get the right medications, especially those who have trouble accessing healthcare. About 40 patients will be split into two groups: one gets extra help from a pharmacist and easy-to-understand education, while the other gets usual care. The goal is to see if this approach is practical and improves medication use in this community.
