Summary
This study is testing whether providing nutritional support to older adults at risk of malnutrition when they leave the hospital can improve their recovery. The program aims to help 2,000 participants get better nutrition after discharge to see if it reduces hospital readmissions, improves strength and quality of life, and helps them stay healthier. Researchers want to know if this practical approach can be successfully implemented in real healthcare settings.
