Summary
This study looks at whether giving older patients who are at risk of malnutrition a special nutrition plan, along with help from a hospital liaison team, can lower their chances of being readmitted and improve their overall health. About 2,000 adults aged 50 and older will take part. The goal is to see if this approach helps them live better and longer.
