Summary
This study is testing a set of tools and training designed to help nursing home staff better manage residents' health issues on-site, with the goal of preventing hospital trips that could be avoided. It's a pilot program involving 12 nursing homes in Belgium, where half will use the new tools and half will continue with their usual care. The researchers want to see if the program is practical to use and if it shows promise for improving care and reducing hospital visits.
