Summary
This study is testing a personalized nursing program designed to help people with type 2 diabetes manage their blood sugar levels more effectively. It will involve 72 adults who will receive tailored education and support over several weeks. The goal is to see if this approach leads to better blood sugar control and improved self-management skills compared to usual care.
