Summary

This study tested whether a special nurse-led support program could help elderly patients stay home and avoid returning to the hospital soon after discharge. The program provided a dedicated nurse who created a personalized discharge plan, educated patients and caregivers, and checked in regularly for four weeks after going home. Researchers wanted to see if this extra support reduced unplanned hospital readmissions or emergency visits within 30 days.
