Summary
This study is testing whether a special care team led by a nurse can help hospitalized adults who have several long-term health problems. The team reviews the patient's medications against the latest medical guidelines and educates them about their treatment. Researchers want to see if this approach helps patients get the right medications when they leave the hospital and reduces their chances of being readmitted or visiting the emergency room in the following 90 days.
