Nurse-Led team aims to boost guideline care for complex patients

Summary
This study tests whether a nurse practitioner-led team can help adults with multiple chronic illnesses get the right medications and reduce hospital readmissions. About 178 hospitalized adults with conditions like heart failure, diabetes, or kidney disease will be randomly assigned to either the nurse-led team or usual care. The team reviews medications based on guidelines and follows patients for 90 days after discharge.
